New Reports in Salaries GC file

New report in Salaries GC file: $$ Report by Teacher.
Accessed via button on Salaries GC menu. Produces a report of all amounts paid during a particular session, or a specified date range, and summarises by teacher. User also has the option to show breakdown by date paid.

Also changed the name of the button above this one to say $$ Report by Session (used to say "Print progressive report by Cost Centre"). This script produces a report of all amounts paid during a particular session, or a specified date range, and summarised by Cost Centre. Each session prints on a new page.
